Message from the Health Coordinator

Dear Parents and/or guardians:

WE MADE IT! Thank you for being so flexible and willing to support our healthcare re-entry plan during this unique and challenging school year. Because of your support, St. John’s was able to stay open the entire school year during the COVID pandemic while most schools went virtual. We could not have done it without you. All praise to God for blessing us with in-person education!

A couple of reminders for the upcoming school year:

UPDATED ENTRANCE FORMS, PHYSICALS & IMMUNIZATIONS

It is Virginia State law that updated entrance forms, physicals and immunization records be submitted before the first day of school. Attached are the necessary forms to complete, sign, and submit to the school office. All may be faxed to (540) 349-8007.

  • An updated Commonwealth of Virginia School Entrance Health Form for 2021-22 (attached)  School Health Form 
  • An updated comprehensive physical exam dated 08/25/20 or later.
  • Updated immunization records (provided by a physician).

In addition to these forms, if your child requires permanent medication that must be stored in the clinic (inhaler, Epi-pen, prescribed/otc medications, etc.), you must submit a Medication Authorization Form. These will be sent closer to the beginning of the school year. If you would like a copy of this form now, please contact the office: (540) 347-2458.

UPDATED IMMUNIZATION REQUIREMENTS

Virginia has passed legislation requiring additional vaccinations for school admittance, effective July 1, 2021. These requirements will affect the 2021-22 school year and immunizations must be submitted to St. John’s school office before the first day of school. All Kindergarten students must submit proof of two properly spaced doses of hepatitis A vaccine (HAV). The first dose shall be administered at age 12 months or older. Most student’s immunization records already reflect this immunization. You may receive an email notification from our office stating that your student needs additional vaccinations if we do not currently have proof of vaccination.

All rising 7th graders must submit proof of their first dose of the meningococcal conjugate vaccine (MenACWY). The first dose must be administered prior to entry to seventh grade; the second dose shall be administered prior to entry to twelfth grade. These vaccinations are a required part of the student’s health record and a requirement for attendance at St. John the Evangelist School. Please submit proof of the vaccination to the school office. This can be done in person, via email, or fax: (540) 349-8007.
